This application will allow St. Joseph Parish to withdraw funds from your checking or savings account for tithing as per the directions you provide.

The first transaction with the bank (US Bank) will show $0 on your statement.
After this initial transaction the correct amount will show up on your statements.

Please print this completed page, sign and date it, and attach a voided check to this application and mail to St. Joseph Parish, 404 W. Lawrence St., Appleton, WI 54911 or drop it off at the Parish Office or in the weekly collection basket.

Changes must be received BEFORE 11:00am Monday if you want it effective that day.
